Salary: $20/HR

3+ years of work experience working with homeless individuals suffering from complex disabilities required

A Boston nonprofit is hiring a Case Manager to be responsible for the specific caseload of tenants. Apply to provide support services to individuals; advocating for appropriate mental health, health and other support services using Harm Reduction, and Motivational Interviewing techniques.

Responsibilities

  • Case Manager will carry a caseload of 10-20 tenants and meet regularly with each individual tenant to address barriers and provide support services to meet needs
  • Work with tenants to create client–centered, strengths- based individual treatment plans to be periodically evaluated and edited as necessary/appropriate and/or according to program standards
  • Perform ongoing psychosocial/clinical assessments and other documentation for tenants in caseload and responsible for maintaining and updating tenant charts
  • Confer daily with other program staff to promote program integrity and to proactively identify any tenant issues that need addressing
  • Educate, coach, and reinforce activities of daily living (ADL) skills, including, but not limited to personal hygiene, budgeting, cooking and leisure time activities
  • Assist tenants in shopping, laundry, and other ADL as needed or requested
  • Meet regularly with each tenant to seek opportunities for ongoing psychosocial rehabilitation
  • Work with the Clinical/Lead Case Manager and Program Manager to evaluate tenant progress
  • Work cooperatively with other agencies to secure needed levels of support for individual tenants, advocate within the mental health system for timely and appropriate services for tenants as needed and requested
  • Guide tenants with severe and persistent mental illness in an intentional progression towards increased autonomy, improved life skills, and enhanced quality of life
  • Involve tenants in milieu to enhance connectedness with life satisfactions and responsibilities
  • Attend team meetings for staff to do treatment planning and activity planning in conjunction with other team members
  • Meet for regular supervision with the Clinical/Lead Case Manager
  • Advocate within the mental health system for timely and appropriate services for tenants as needed and requested
  • Accompany tenants to outside appointments as needed or requested

Requirements

  • Associate’s Degree in psychology or other related field such as counseling, psychosocial rehabilitation
  • 3+ years prior work experience working with homeless individuals suffering from complex disabilities 
  • Familiarity with the concepts of psychosocial rehabilitation and consumer empowerment
  • Experience with case management and implementing treatment plans
  • Knowledge of treatments for chronic mental illness, including medications
  • Previous experience working with dually diagnosed individuals
  • Experience assisting people with ADL skills
  • Experience working with other vendors and services within the public mental health system
  • Previous experience moving homeless mentally tenants into housing
