Temporary Case Manager

A Boston human service agency solely focused on ending childhood and family homelessness is seeking a Temporary Case Manager. Apply to provide housing search, placement, stabilization, and 2-generation services to precariously housed and homeless families with children.
Responsibilities
- Case Manager will assess families for strengths and needs related to housing, employment, health financial literacy, and children’s social-emotional development
- Develop a plan to best meet goals defined by family, staff, and partners
- Provide ongoing case management to promote families’ long-term stability in permanent housing and children’s long-term healthy development
- Refer to and actively obtain enrollment of participants in critical community-based resources
- Engage program administrators to ensure continuity of services and plan to maintain long-term housing stability and children’s resources
- Maintain clear, non-judgmental documentation in clients’ files
- Complete data entry and other documentation on a timely basis that meets contract expectations
- Participate in weekly supervision, agency training programs, and professional development as assigned
- Maintain confidentiality of all client files and information
- Develop and implement care plans, housing stability, and SEL program participation
Requirements
- MSW or Bachelor’s Degree and equivalent experience required
- Professional social work experience
- Bilingual in Spanish/ Bicultural background is strongly preferred
- Excellent written and verbal communication skills
- Demonstrated success in applying evidence-based practices including Trauma-Informed Care, critical time intervention, and motivational interviewing
- Professional experience with housing, schools, and hospital settings is highly desired
- Ability to work in both office and field-based settings with a diverse, homeless population
- Flexibility and compassion essential
